My Health Portal

Access to your health information is important. My Health Portal is an easy way for staff to electronically share your health information with you – and you can view it at your convenience, anywhere from an internet-enabled device.

With My Health Portal you are able to:

  • View your health records

  • View your lab, x-ray, and other test results

  • Store important health information

Sign In

Not signed up yet?

  1. Let the front desk team know you’re interested in signing up.
  2. Once you provide your email address, we’ll email you a sign-up link. (The email will be from no-reply@healthdataexchange.com, so check your ‘Junk” folder if you don’t see it in your Inbox.)
  3. Once you create an account, you can log in to view documents or messages we have sent. You’ll receive an email notice when you have an item to view in your portal.
  4. If you want to forward your records to another physician, simply click the “Forward” button and you can email that document.

This is a free service to offer you more convenience and access to your health records.
